Job Summary
Make a difference in the life of a child! Home visiting RN will provide ongoing health supervision for low-income pregnant women and families with children 0-6 to ensure healthy outcomes. Position reports to the CHIP Team Supervisor and collaborates with Parent Educators to provide comprehensive service delivery to families on caseload. No night/weekend/holiday nursing shifts!
Responsibilities and Duties of Home visiting registered nurse
Essential Functions:
- Initiate and maintain regular and long-term contact with assigned families.
- Provide direct services to prenatal and postnatal women and families in the family’s home to ensure proper use of medical home, provide health education and support.
- Provide family-centered, strength based interventions as required.
- Maintain case management documentation and required updated reports on families served.
- Act as a liaison between the medical home and the family. Accompany and/or provide transportation for families to medical appointments when necessary.
Additional Requirements for Home visiting registered nurse: Knowledge of and expertise in providing home-based services.
- In-depth knowledge of fetal and child development, human sexuality.
- Strong interpersonal skills and cultural sensitivity.
- Current driver’s license and willingness to use own mode of transportation.
- Ability to work well within a team
- Ability to establish and maintain programmatic boundaries while being flexible and supportive of families.
Qualifications and Skills for home visiting registered nursw
Education/Experience:
- RN or BSN required w/current Virginia State License as a Registered Nurse or Licensed Nurse
