JOB SUMMARY
The Case Manager, Registered Nurse (RN) is responsible for organizing and directing hospice care services and is experienced in nursing with emphasis on community heath education/experience. The Case Manager uses the resources from the community to plan and direct services to meet the needs of individuals and families within their homes and other care locations. Job responsibilities may include but are not limited to:
- Completes initial assessment of patient and family to determine hospice care needs.
- Regularly re-evaluates patient nursing needs.
- Initiates plan of care and makes necessary revisions as patient status and needs change.
- Counsels family and patient in meeting nursing and related needs.
- Provides health care instructions to patient as appropriate per assessment.
- Communicates with physician regarding the patients needs and reports any changes.
- Participates in on call duties as defined by policy.
- Respect others by treating them the way we want to be treated.
- Demonstrate kindness, compassion, honesty, and integrity at all times.
- Help create a culture of superior customer service.

REQUIREMENTS
- Clear and active Registered Nurse, RN license in North Carolina
- Graduate of an accredited school of nursing
- 2 years of Hospice experience, preferred
- Clear and valid Drivers License
- Current CPR
- Supervisory skills
- Successful criminal background check
- Subject to pre-employment drug testing
- Willingness to travel 75% of the time
